Where can I find a list of questions to ask when visiting a long term care facility for a family member?

I am taking my mother-in-law to visit an Ontario government subsidized facility for her husband who has dementia and is in need of long term care immediately. Where can I find a printable online list of questions to ask when we go for our tour, to make sure they will be able to provide the care my father-in-law needs and to make sure they are following Ontario by-laws.
